If you suspect neglect or abuse in a California nursing home, you can file a complaint with the California Department of Public Health (CDPH), which oversees licensed health care facilities through its Licensing and Certification Program.
Ways to File a Complaint:
- Online: Submit a complaint via the Cal Health Find portal.
- Phone: Call the CDPH Complaint Hotline at (800) 236-9747. Centers for Medicare & Medicaid Services
- Mail or Fax: You can draft your own letter or use the CDPH complaint form and send it to the appropriate district office by mail or fax.
- Long-Term Care Ombudsman Program: For advocacy and assistance, contact your local ombudsman through the California Department of Aging or call (800) 231-4024.

In California, Adult Protective Services (APS) agencies operate in every county to assist elder adults (60 and older) and dependent adults (18–59 with disabilities) who are unable to meet their own needs or are victims of abuse, neglect, or exploitation.
